Something as simple as having a single page with all the transpositions listed in a grid for the root keys can help to reduce the finger-counting I used to do so much of in a panic. You just have to learn to think on your feet, know a lot of bar chords, and get a feel for when a capo makes sense and when you need to figure out some new positions.
